WealthPath is a cohort-based homeownership journey designed to help individuals and families build long-term wealth through homeownership. The program combines financial education, one-on-one coaching, and a pathway to attainable homes in Ascension communities.

This brief form helps us learn more about you, your goals, and your current readiness for homeownership.

The program includes monthly evening sessions over a 12-month period, along with optional coaching and resources to support your progress.

Confidentiality and Information Use Disclosure

The personal, financial, and demographic information collected through this enrollment form is used solely for purposes related to program administration, participant eligibility, impact measurement, and reporting to supporting partners and funders.

All information provided will be treated as confidential and will be maintained in a secure manner consistent with applicable privacy and data protection practices.
